has anyone worked as a CARE manager for Senior Bridge?

Specialties Home Health

I got an email from a recruiter for care manager positions with Senior Bridge in my area. I've looked at their website & I think it's something I may be interested in. It's not home health care per se, it is more interviewing and documentation. The point is to prevent hospital re-admissions.

Just wondering if anyone can share personal experience with this kind of thing. Thanks!

If you applied with an agency and didn't go directly through the CMNetwork through seniorbridge it is up to the agency if your name is already in Humana's system as working for an agency

Specializes in Geriatrics.

If its too late do I work for the agency for 6 months and then apply or do I have to quit for 6 months?

You have to quit. I'm not sure if it is 6 mths. or a year. It might only be 6 mths. You will have to check on that.

I finally got through to someone and got the info I needed. (MODERATOR EDIT OF NAME - PLEASE DO NOT POST NAMES OF INDIVIDUALS PER TOS) was VERY helpful! Humana will pay $300/mo per member you see. That would include the initial survey and an additional 2 or 3 more visits. That is definitely way more than an agency would pay!!!

They need a release from the agency if you already signed (which I did) or you have to start 6 months after quitting. I am going to compose a letter to my agency today to request that release. I hadn't even begun working with them but already had a few issues of concern so I expect they will release me in order to avoid trouble with Humana. (?) The release can be an email or letterhead. I forgot to ask if the email needs to be addressed to Humana or not but will email (MODERATOR EDIT OF NAME) today to ask.

Did she quote you exactly $300? Do they pay different amounts for different areas? I'm asking because my pay is less than $300

Ok.. I just found out that the $300 rate is an RN rate. I also found out that they are no longer recruiting LPN's

Specializes in Geriatrics.

How do you start an agency catered to this?

Have no idea. That's a question for a recruiter.

I should have read this before submitting my application to an agency. I haven't started but I have sent in all my information and picture.

See if they will release you

I did submit my resume to Humana. My biggest concern is that my state isn't listed on the website as a location. I'm wondering if there will be enough clients to allow me to quit my job. I work 5 days a week now. I can't really see doing both. Ideally I'd like to work less than I am now.
