I got myself in a predicament. I visit a facility and the nurse was venting to me that she disagrees with the way the family is treating the patient. Says that the patient is "loaded" and the family is trying to hurry things along in her Hospice care. So the nurse tells me that she is NOT giving her scheduled risperdal and charting that she is indeed giving it. So she is lying. It irks me but not enough to throw her under the bus. Eeeks. And so I was telling my social worker, and now it has to be reported.
So what is gonna happen now? Will I have to be questioned a zillion times? This nurse said this in front of the care plan coordinator and 2 other people, we were all in a room.
How do I get myself in these messes? What is going to happen? My director is to meet with the facility today. I haven't slept all night.
i believe she was trying to protect her pt from being overly sedated.i just wish she had notified the dr., explain situation, and ask for order to hold.
i've seen too many times, families purposely snowing pt. (i can't prove anything further.) and it has looked suspect.
the nurse was wrong and careless by venting to strangers.
at that time, she was her own worst enemy...
but i understand the incredibly frustration she was feeling.
there are soooo many different dynamics w/pts and family in hospice.
a lot of it is sad, a lot of it is scary, and a lot of it is immensely frustrating.
had it been a colleague, the likely response would have been, "i don't blame you...but why not call dr. x".
i feel sorry for the nurse...and the pt. (unless that is what pt wants.)

Well, my director met with the DON today. Contract has not been cancelled, but I imagine we won't be getting any more referrals. We are keeping the patient. I dread going back into that facility!!
OP: I am sorry, it does appear that I misunderstood your post! I thought you were "a visitor", I now gather you are a visiting nurse! Sorry if I sounded rude before!!
No worries! I figured that, and changed my present place of work to Hospice. I have been a Hospice nurse for about 8 months now. Still have a lot to learn.
I was confused at first too, then I saw you mention your director so I figured you were some sort of outside contractor rendering a service. Just keep your eyes down shoulders back (isn't that what they say for prison? LOL). YOU did NOTHING wrong! Sounds to me like there really is some major moneies involved.....good luck :)

Cannot trust the facility because they are not truthful because they say they gave the med but didn't. How do we know how to treat the patient if we don't know what the patient is getting? And the facility just does what they want imposing their views onto the care and not following the plan of care. Some examples are that the family wants the risperdal given and even asks for the MAR to see if it was given or not. Another example, facility *thinks* she needs sunshine, when patient states that the light hurts her eyes, so daughter wants the window shades closed and lights off, because this is what the patient wants. another example patient and daughter want soft soothing gospel music, the facility hates the music and turns it off because it gets on their nerves.
Order was written on the chart not to get patient out of bed, because patient is very uncomfortable plus very drowsy plus very anxious. Same nurse disregarded the order and was gonna get patient up, until daughter caught them and raised cain about it.
Update on the situation: I guess today the facility is changing their story and the 2 nurses that heard the other nurse say this are changing their story to say they heard her say something different. They are saying that she "she just tells the daughter that she gave it when she didn't" and it has nothing to do with the charting, she charted truthfully and then they decided to tape their statements.
This nursing home also believes that the daughter is trying to hurry things along by holding patient's meals if she pushes the food away. And also the fact that the daughter would prefer her not to be in a lot of anxiety and pain. The nurse stated to me that the patient is "loaded."
So would you have went to the DON first? I didn't realize the magnitude of everything and told our social worker who told our director. Who by law has to report falsifying medical records to the state.

They are saying that she "she just tells the daughter that she gave it when she didn't" and it has nothing to do with the charting, she charted truthfully and then they decided to tape their statements.
This place needs to be reported......They've changed the charting to reflect her statements about holding the meds.....yet they are still admiting to lying to the family. I would be very uncomfortable with this place.....and they arbitrarily decide to change treatment according to thie whims and beliefs.....I smell a rat.

Their logic boggles the mind. It's "okay" to lie to the family as long as the medical record is accurate?! WOW.
