My client was an 18 year old male who accidently shot him self in the lower right leg with a rifle. He had a wound vac in place. I'm writing my first care plan off him. I have a nursing diagnosis of "risk for ineffective tissue perfusion related to large wound on right lower extremity." I have one goal writen "signs and symptoms of ineffective tissue perfusion will continue to be within normal limits as evidence by pulses, skin color, blood pressure, and capillary refill by date of release" But i'm having trouble coming up with another short term goal and a long term goal??? If any one could give me suggestions that would be great. Also the client expressed concers of impaired mobility and how well he would heal and how long until he would be able to ingage in physical activity. I'm not sure how to write that diagnosis?? Maybe something like "mobility and activity concers related to right lower extremity wound and wound vac as evidence by client stated concers??" Any help would be great! Thanks

Is the risk for diagnosis supposed to be your priority? I only ask because this patient has actual problems that you could list like impaired skin integrity, activity intolerance, acute pain, etc
As far as the questions he asked you could do knowledge deficit but you also might be able to cover that topic in the activity intolerance too.
i'm not sure on what should be priority. I'm writing 3 nursing diagnosis so they are all covered. Thanks for the knowledge deficit, i never thought about that!

Impaired mobility and Acute pain would be my top priorities with this patient if he were on my floor.

My first thought was risk of infection with the wound vac.
