Hello I am new here and sorta new in the field.. I was supposed to give a patient an injection and gave a PPD medication instead of the flu vaccine and then i didnt realize it again on another patient! Both patients had bigger arms..but only one i was aware when the patient was still in the office! The dr said they she will be ok. So i reported one instead of both. my friends say she should be ok.. but i still have a bad feeling, besides the fact i messed up.

You should of just not said anything ... I'm sure they are not going to die over an I'm injection of PPD..lol...you have to cover your own behind or it's gonna be a long hard road. Do follow six rights and double check what your drawing up ...just my own opinion

What about the flu shot the person thought they got and didn't?

What if that person gets the flu (because they didn't get a flu shot that they thought they did) and dies from complications of the flu?

Seriously? You condone lying? And do you have information to prove or even suggest "they are not going to die" or be harmed?

And of course since reporting an error is not about YOU, it's about the patient, you can't decide not to report the incident with the second patient just because it's a lot like the first error.

And your friend who says, "Don't worry about it," is she a real licensed (RN LPN/LVN) nurse? I can''t believe a real nurse would say anything of the kind.

Like this a bunch of times!! I think this is going to be a long thread.

I would fess up about the other patient. It is important that everyone knows the error.

I would also look up all the possible complications of giving PPD IM just so that I can have that in my arms of information and to be able to educate the patient.... And also to make sure that I remember this error and learn from it!!

I deal with PPD, Flu vaccine, Hep B series, Pneumovax etc almost daily, and a lot of the times a patient will be receiving these from me in one visit....

With all those vials on my table I always double and triple check myself and my five rights. I make sure I have the right med, put in the right syringe, being given in the right spot for every single one I'm giving ONE AT A TIME! Don't ever rush and always read your labels!

Fess up, make sure your patients are ok and learn from this.

Are you serious??!!

Please tell us you're not really a nurse or student nurse. We don't appreciate being pranked, but I'd rather that were the truth than worry that you're actually taking care of patients with that perspective.

Yikes! If this is how you feel about this error I wonder what you'd say to a Dig error or Heparin drip or even a Mag bag mishap.

Errors need to be reported. Not just for pt safety which is first priority but to also see where in the process are we lacking that way we can change our approach.

I'm all for CYA but if we're following our tools and practicing integrity the road is a bit nicer. And I'd sleep better at night.

You can't possibly be serious...of course you report your error....so the patient that thinks they got a flu vaccine gets the flu and dies...who's fault will that be?

NEVER lie and NEVER cover up mistakes.

Um, Didnt say i used a tb needle. i used an IM NEEDLE 25 REGULAR NEEDLE. I JUST mistook the medication.

You have to report the other error. Just suck it up and do it. The longer you wait, the harder it will be.

Could this be a medical assistant without knowledge of the rights of medication administration. In some "doctor's office", the qualifications for administration of medications, is low pay. If so, was this reported to the physician whose license your working under. What does he/she think about this?
