Hi! I'm a bit confused with the CT-Nur form. I am an RN in the Philippines, do I still need to fill up and submit the CT-Nur form even though it will show in my CES that I am licensed in the Philippines?

And how did you fill up the code numbers like the School codes etc.

I did not understand the CT- NUR FORM thing. Dose it mean I should send it to the licensed agency that processed my license back home and then send it to the BON in Illinois? If so, I already sent form from my agency to CGFNS. Please help I got confused

you could see the CT-NUR form here (2pages). you have to fill it up then bring it to PRC along with the photocopy and original TOR and then photocopy of your PRC ID. then PRC will be the one to mail it to CTS.

should i wait for CTS' confirmation? or i'll just proceed on paying on pearsonvue?

    thanks guys!

Are you sure that the PRC is going to be the one to mail the ct-nur to the cts??? I think we need to be the one to mail it to them, together with our 4page application form and the receipt from credited vendor.

It's a double application, so you can register on both to save time.

I had the same problem. I had PRC filled the CT-NUR that i downloaded from Continental testing and PRC gave it back to me cause it says "return to the applicant".Then later i found out that PRC should be the one who will send the CT-NUR in IL. The form from the IDFPR website states "Do not return to applicant". I was confused but what I did is, I just disregarded the former and applied the one from IDFPR (again) in PRC and I had it rushed so Continental testing received it in like after 3 days. So what I'm saying is, i think PRC is the one who will send the CT-NUR not you...
