Name not changed after short marriage, how to answer Application to boards of Nursing

Good evening Nurses,

I had been married for a short time (a year and half), I did'nt changed my name orused his last name in any document even after marriage, should I say yes to the question, did I ever changed my name? I could submit a copy of the divorce and marriage license. I'm just not sure if I should say yes or no. Thank you so much!

You did not legally change your name, so the answer is no. Why would they want know? They do a background search to make sure you are in no way ineligible for what ever it is you are seeking.

Huh! I am confused. Doesn't matter how long you were married, if you never legally changed your last name then nothing needs to be mentioned. Did you ever change your name? No!

When I got married a million years ago, I never changed my last name. I kept my last name that I had since I was born. It is my identity till I die. After my divorce, I never had to worry about changing my name back.

You never changed your name so why would your answer to the question "did you ever change your name?" be anything other than no? They just want to know if they need to background search you by another name.

If you never changed your last name, the only logical and correct answer to this question is "No."

The board of nursing doesn't care that you were briefly married once upon a time. The name question is strictly for the purpose of thorough background checking. Good luck to you.
