Yesterday i received a letter from the BVNPT after more than 2 months waiting about the Declaration of Nursing Education and or Licensing.The questions here are confusing. Can someone here who got the same letter if you can help me fill it up please?..:(

I was at a friend's BBQ party this weekend, I met someone there who was of PH descent, but educated in CA. She got her LVN license almost a year ago and still looking for a nursing job, she said it's very rough. Many of the hospitals or nursing facilities hired from within, that is to say, many that did their clinical rotations at a particular place was usually hired in, they are called "internals".

She wasn't picked from a field of 18, only 3 openings.

Anyways, I asked her about this "declaration". She says everyone has to fill it out, it's to verify that what you claim on the document is the absolute truth or if the BVNPT finds out at any point, that one was dishonest in their answers, they could take away the license at any time or not issue the license at the start.

There are some schools in CA that offered classes on-line or was taken at another location, meaning that there is a chance that they took a course in one city and may or may not have taken the clinical part not associated with the school. There are some schools that don't make the BVNPT standards and so if it's listed in the declaration, they will contact the hospital to verify that one actually did their clinicals there.

They will also use any information provided to make sure that it matches up with what your transcripts say. So we need to be real accurate or it can cause a application delay or denied.

She says that she thinks the BVNPT added more sections about the governmental agencies to filter out the international applicants. It's not only because of the PH ones, but she says there are so many grads coming in from China, India, Nigeria, Mexico, Russia and the UK. Many of the applicants are also trying to submit false documentation so the declaration is needed to be used a written proof if there's any problems to not grant or to not renew a LVN license. That's what is stated if you're signing the declaration.

She has a cousin who went to school in PH and got her BSN and been hearing all the horror stories of the CA BON mass declinations of the nursing applications. She had plans to attend a top PH school to get her BSN, but knows now it's not worth the expenses and time to get it. I told her no matter how hard or expensive it is to get into a US school, don't bother thinking about trying to get into a PH nursing program, it will only bring nothing but heart ache and major disappointment, if she plans to be an RN in CA.

hi i just recieved also a letter from BVNPT lastweek a Declaration of nursing education and/licensure and i dont know how to fill this up either...it makes me feel so upset why they makes this thing hard for us,we are already RN in our country(Phil).In questions#5 Name and address of the hospital(s)where you completed the clinical portion of your nursing education..I have trouble answering this question cause i was gratuated year 2007.i had no list of clinical rotations and that was 5 years ago so i cannot provide the name of the hospital,instructor,add,tel,fax,date started and completed and no.of hours in this questions.what should i do?..can i send them my all cases instead?....please i need advice nurses...i would really appreciate it!

Hi guys, I just received my mail from the Boards today asking to complete the declaration of nrsng education form, yet I don't know how to answer #5, :(....If you guys don't mind helping me, I was wondering if what are your sources in answering # 5 **Date started, **Date completed, and **Numbers of Hours Worked. I was thinking If do I need to call my school for that? And for #8 is it ok if I would send them a photocopy of my expired PRC card and some photocopies of certificate that I passed the NLE in Philippines even its not certified true copy?....any response would be very much appreciated....THANKS..

@ mjhay...hi kabayan,I think certificate from the PRC(the diploma type thing) will do even if its photocopied its like what we send to CABON also..I havent receive my decl.of nursing educ.yet but i think its somehow like our forms of clinical rotation schedule,name of hospitals where we had our duty..and not the cases ha..and i just wanna know also if did you submitt Record of Nursing program to your school?

@ hurty..better contact your school and ask them to give you a copy of your clinical rotation,not the copy of ur clinical cases,,they only need the hospitals name where you had your duty..etc..

@proudpinay,.,yes I was able to complete all their requirements needed for the application, I submitted the sealed envelope that has TOR, Record of Nursing Program, and RLE in it,...and I was surprised that they state in the letter that the Board is requiring me to have them my sealed Record of Nursing Program as to continue my application process,..I was thinking then probably that they just overlooked on my papers, and they also state that they had received my Transcript from Registered Nursing Board,..but they need my Record of nursing program to complete the process as well as the declaration of nursing educ...anyway I should call them tomorrow for any clarifications on why they didn't have my record of nrsng program, and to ask them about the declaration of nursing, i still don't know what to answer though,...thanks proudpinay, ill keep u posted on what info i may get tomorrow..Godbless to all..:)

I sent my LVN application a month ago and now seeing this makes me a little worried. It feels like my window of opportunity is getting smaller and smaller in cali. =/ well, at least people are not getting denied like our RN applications but its always those extra documents that makes it a hassle. I haven't received the declaration letter yet but i know its coming. Please keep everyone updated with this situation. I know this thread will help a lot of us out =) Good luck to all of us!

seeing all these gives me a scare .. looks like it's as hard to take the lvn exam..

can you help me what site can i get the form "declaration of nursing education/ or licensure" for BVNPT... so i can include it the requirements that i will submit to the board....

the declaration form will be sent by the BVN after your initial evaluation. for now just send the required documents stated on the form of BVN so that they can do your initial evaluation.

thank you proudpinay..i already sent my form of declaration of nursing but i did not provide the date started and date completed and hours because like what i said i have no list of clinical rotations niether the school.i already sent nursing program form in the school. i dont know what will be their response,maybe im not eligible=(

how come we have to submit a high school diploma? i only receive a letter stating to complete the declaration of nursing education, do i still need to submit a high school diploma even though it doesnt state to submit? im confuse..
