VANCO in 250cc bag or OK to buretrol?

Hi all-

So far we have been mixing Vanco with 150cc in the buretrol and running it over 1 1/2 hrs on my unit. A friend from another facility told me you had to mix it in 250cc- so what's the deal?

On our unit, policy is to mix Vanco 1 gm in 250cc's and run it over 2 hours via a pump to avoid giving it too fast and causing red man syndrome.

If you check with your facility pharmacologist, I'm sure that would clarify the issue.

Same here .. always seen it mixed as above. and I work agency so i work in different hospitals in the area.

Since I'm spoiled and the pharmacy mixes our drugs for us.... the standard for the mixture is 1gram/250ml..... BUT if you have a patient that is recieving lots of fluids, and is at risk for fluid overload, they will mix it in 100ml.

We get Vanco premixed from pharmacy in a 100 ml bag. Drug book says to give it over an hour.

Same here. Vanco 1g in 100ml saline and running it over 1 hour minimum.

This just made me think..We are always searching for IV pumps when its busy and can never find enough, I could just imagine having to find a pump for everyone that gets vanco! :uhoh3:

We are supposed to mix it in 250cc D5W, but will mix it in 100cc's if the patient cant tolerate the fluid.

On a side note, I have never seen any of my patients develop "red man syndrome". Does this really happen?? Or is this a legend from the textbooks.

Have had several patients with Red Man Syndrome. One patient with only 25cc over 15 min of Vanco 1gm/250CC. Doctor SWORE I had run it in too fast.

Ran it slow as pt had allergic HX.

Tx with Demerol...had to show doc my Nursing Drugbook for TX of this reaction. :rolleyes:

Yep, someone opened up the drip on my son, and not only did he turn red, he immediately had difficulty breathing- was taken back in OR to be intubated- Scary but everything turn out O.K. - well they delayed his surgery for a few weeks.
