Does anyone have a template on a letter to send a LVN IN TEXAS THAT HAS over 100 outstanding notes since February.?.. What's the process to report to BOn.

Is this in home health? I'm not sure I understand the circumstances.

Speaking generally, willful noncompliance with documentation standards can certainly be addressed within a company/agency/facility's disciplinary policies.

Did you check TX BoN for reporting guidelines? Did you follow up with company policy re:disciplinary action?

It is a home health... We have done everything in our company policy to try n get the late paperwork On since February with no luck

Consider if Medicaid/Medicare notification is mandated, as if you are audited and there is no documentation the agency will be cited as possibly billing for services not provided. There was risk of fines & reversal of payments. Plus if the corrective action isn't documented then the agency risks losing accreditation . Check the BoN for reporting guidelines. I've seen BoN's issue sanctions for failure to follow standard of care & documentation.

See if you can get your atty to write her a nastygram "inviting" her in to the office to sit down and complete her charting and perhaps implying that the BoN might not get notified about her breach of standard if she does it by X date (less than ten days away). At least if you get cited for bad documentation you will have a paper trail of how hard you have tried to get her to do the right thing. I assume you have already fired her sorry butt.

Then you need to seek corporate guidance on this matter and speak to the BON. I would not be seeking advice or a form letter on an public forum. Are you a RN?
