Two weeks ago I took a very simple practical test to evaluate my competence skills in obtaining vital signs. I got a score of 10.65/12

On Monday my lab teacher emailed me and asked if I had my vital sign checks off paper. Two days later, I emailed her back and told her I had my vital signs check off. She told me to bring it with me for the next class. I told her would do that. Thursday morning on 12/8/16 I went to school and I checked my bag big could not find the paper. I thought I had left it in my car. When my partner arrived at school I asked her to let me see her paper and I told her maybe I left my paper in my car since I couldn't find my paper in my bag. That day I didn't drive my car. I had to find someone to drop me to school.

My class teacher asked me to give her the paper and followed me inside the classrooms and at the lab. I finally told her I don't know where I did put my graded paper. Later on, I told my lab teacher that I may have left my paper in my car. She told me if I found the paper just take a picture and sending to her.

I couldn't find it in my car. So I recreated it as best as I could with the same grade.

My lab teacher gave me the paper back, including two other students. The rest of the students had their grades recorded. Ours weren't.

My lab teacher asked me and the other students to bring the paper back because she had forgotten to record the grade. I can not find the paper. She initially verified my grade with my group member partner who attested that my grade was 10.65/12.

Under pressure to find the missing graded paper, I was also able to recall each itemized grading from memory accurately and submitted it to my professor. She won't accept that too.

I am worried that she refused to accept it even though she is the one who forgot to record my grade.

If she doesn't, it could cause me to fail the entire class. This grade could be the one that breaks the camels back and I'm not willing to take that chance an not have this grading included in my final.

I dont have the paper anymore and I have looked everywhere. If I knew she would need the paper back, had she indicated that all past graded papers were permanently essential, I would have surely kept it. My mind was focused on the upcoming head and toes assessment and this issue has been very mentally draining and destabilizing my final studies.

My question is should I email the Dean about this issue? I am very scared of the teacher, because I don't know if I do that she may fail me in the final exam.

Don't panic. Based on the information you provided, this issue arose due a mistakes by your instructor who didn't record your grade, and you - who didn't keep the paper copy of your checkoff. IMO, it is up to the instructor to fix this because s/he is primarily responsible. Maybe you could repeat the checkoff? But - sit down and talk about it with your instructor to explore the available options. Don't be defensive or argumentative. For goodness sake, do NOT try to undermine your instructor by 'going over her/his head' because I promise you, it will not end well.

Thank you, but I already talk to her. She told me, I needed to bring the copy. She is not willing to give me my grade or retest me. Based on this information, what advice can you give me?

This situation can go both ways. Like the PP indicated, it is the fault of your instructor as well as yours. I am not in the nursing program, but only taking pre-reqs, but now having read your post and I feel like I might have been in the same situation. I was taking chem lab and each class we, of course, had to conduct an experiment. The following class we had to bring in the complete lab report. Then, the lab instructor graded it, handed it, and I thought of disposing it. However, I decided to keep it just in case. And thanks god I did! The last but one class the lab instructor told us to compile all of the lab report in a nice and cohesive lab report manual so all of the lab reports were in one place. For doing so she said she would give us an extra credit. I was so relieved I had kept all of my lab reports and now, gotten that extra credit.

The point is, always safeguard your college papers, be it a lab report, a quiz or an exam. You never know how things will turn out and you may need them at some point. I do hope that your situation will be resolved in your favor. Good luck!

P.S. You may go and speak to the Dean, but I am afraid it may go back to you and haunt you. Make sure you think twice before bringing this up to the dean's attention. I do not know what kind of a person your professor is, but he/she may take it against you. But like I said, think twice.

No...this is my first semester in the nursing program.

And you only have one nursing faculty member? Counting lectures? I find that hard to believe.

From now on make sure you keep all your graded papers until final grades are fully submitted and on your transcript. I'm sorry you are learning this the hard way- you are not the only one to have this issue.

When receiving my first degree prior to nursing I had so many issues with my grade being inaccurate in a class. The issue was a TA that I never even saw or knew- her job was just grading. I had to constantly go over my grades and get the professor to correct things. Points on a test would be added up wrong to my detriment, I'd have 10 points taken off for something that should have been 3 points, a 100% on a lab report was recorded as a 0%...I could go on and on. I didn't even know the lab report was recorded incorrectly until my instructor was telling me my final grade and I was shocked- he then said I had a 0. I went out to my car where all my class papers were and took my 100% lab report and showed him- my grade would have been vastly different if I hadn't so carefully kept track of everything.

I would email your instructor, so you have a paper trail, laying out what happened. I would have someone else read the email before sending it to make sure it is not interpreted in a way you didn't mean. You could say something like "From my understanding my grade was not recorded after the compentcy as other students were. I have been unable to locate my paperwork and I was not told upfront that I was required to resubmit my paperwork for my grade to be finalized. As neither of us can locate my paperwork I am formally requesting a chance to repeat the compentency."

They have many facilities but I don't have relationship with them.
