    Two weeks ago I took a very simple practical test to evaluate my competence skills in obtaining vital signs. I got a score of 10.65/12 On Monday my lab teacher emailed me and asked if I had my vital sign checks off paper. Two days later, I emailed her back and told her I had my vital signs check off. She told me to bring it with me for the next class. I told her would do that. Thursday morning on 12/8/16 I went to school and I checked my bag big could not find the paper. I thought I had left it in my car. When my partner arrived at school I asked her to let me see her paper and I told her maybe I left my paper in my car since I couldn't find my paper in my bag. That day I didn't drive my car. I had to find someone to drop me to school. My class teacher asked me to give her the paper and followed me inside the classrooms and at the lab. I finally told her I don't know where I did put my graded paper. Later on, I told my lab teacher that I may have left my paper in my car. She told me if I found the paper just take a picture and sending to her. I couldn't find it in my car. So I recreated it as best as I could with the same grade. My lab teacher gave me the paper back, including two other students. The rest of the students had their grades recorded. Ours weren't. My lab teacher asked me and the other students to bring the paper back because she had forgotten to record the grade. I can not find the paper. She initially verified my grade with my group member partner who attested that my grade was 10.65/12. Under pressure to find the missing graded paper, I was also able to recall each itemized grading from memory accurately and submitted it to my professor. She won't accept that too. I am worried that she refused to accept it even though she is the one who forgot to record my grade. If she doesn't, it could cause me to fail the entire class. This grade could be the one that breaks the camels back and I'm not willing to take that chance an not have this grading included in my final. I dont have the paper anymore and I have looked everywhere. If I knew she would need the paper back, had she indicated that all past graded papers were permanently essential, I would have surely kept it. My mind was focused on the upcoming head and toes assessment and this issue has been very mentally draining and destabilizing my final studies. My question is should I email the Dean about this issue? I am very scared of the teacher, because I don't know if I do that she may fail me in the final exam. >
