Go online and do a search for a notary public in your area. They're everywhere ... banks, out of their houses etc... They charge starting at $5 or more to verify a signature on a document or what you need. You need to have picture ID and another piece like a SS card or birth certificate. They usually want 2 pieces of ID but the one they really need is a picture ID with your signature on it.

Not sure where you're located, or what those forms are, but my local library does it for free, and the Currency Exchanges do it for $1 each.

There are also notary publics at courthouses, at least in my experience.
