DO YOU ALL DO WEEKLY, BIWEELY VS AT YOUR FACILITY?

jUST WONDERING??

We do it weekly with the shower schedule. (Yep most residents only get one shower a week, but thats a different thread)

Then we have 3 days all 3 shifts for admits, readmits, and incidents. Q shift temps on all antibiotics until completed and vitals on anyone prn with conditions changes.

VS. are done qshift for 3 days after admit, readmit,or fall. During AB treatments it is done qshift .If they have a change in condition then it's qshift and inbetween . VS are also done with monthly summaries.

At the LTC I use to work at it, it was every week.

Our residents get a shower on Monday & Thursday, Tuesday & Friday ,Wednesday & Saturday, and then vital signs are done all on Sunday and I hate it but what are you gonna do?

Most everybody gets BP meds so the BP is everyday. Before someone broke my thermoscan (thermometer, did I spell this right) I was taking all temps everyday. For the folks that are blessed with normal BP we take vitals every month with the monthly summaries. Medicare is every shift any one with a change of status frequently. :)

speaking of vitals for meds....the only thing I see are apicals for dig...what bp meds are you taking vitals for?

Michelle126,

there is a lot of meds that we do vitals for ... just about all the meds that the elderly take will effect the bp, and it needs to be taken before the med is given . our normal for holding a bp med is if the systolic is less than 100 and the diastolic is less than 60 so for a bp of 98/60 we would hold all bp meds . i can't think of them all off hand that we do this for , lisinopril is one ,,, lopresser is another one .

Just wondering what everyone else does. I haven't worked days in a while (most of our bp meds are given in the am), but remember how long it took me to pass meds. I can almost bet you that no one is taking bps on our days. Do you have a space for it on the MAR? We document under our inital for the med.

Yes there is room under our initial for the bp to be written in .

Thanks everyone! We are supposed to do VS every Sun also. I think had been the only nurse doing them until I said something to the other nurses.

There are many BP meds that are supposed to have BP's taken before hand. I can see where it would be almost impossible to do them before. It would take along time to do them all and get the meds done within the alotted time. Thank God for med aides. I don't know what I would do without mine. I truly think I would leave LTC.

Med Aides ? we don't have med aides . It would make getting the meds out on time so much easier. especially with all the bp's that need taken and then don't forget about the interuptions that we have.... families calling , doctors calling ,, doctors making rounds,, not to mention helping the resident .. for which we are there for in the first place. Then to have to get the meds out inbetween therapies and activities.
