I'm new at this.....so here's my guess. Patient history and dx, vital's and lab values, med's and competency level. What do ya think??

Also....this could depend on the situation and who is doing the reporting......

Could it be family doing the reporting or could it be ED staff reporting to floor staff or one nurse to another during shift change?

Or it could even be the patient him/herself and/or family reporting to an ambulance/paramedic crew.

I was recently taken to the ED by ambulance and was asked alot of questions......so I did some reporting myself.

Some questions I was asked were name, did I hit my head, am I hurting anywhere besides the arm, my age, allergies, menstrual history, and many more questions, but I think those were some of the very first.

This is what I want to know in LTC

Name, current dx (why are they here), LOC, Pills whole or crushed (swallow difficulties). Then I might ask about last pain med for certain res/ dx. I will also aske...any IVS or weird treatments/ times.

Of course this is sooooo different because I'm in LTC, but since I only work a few shifts a month, things change and the pts are different every time I work.

This sounds like a homework or a test question. What does your instructor &/or textbook say?

But it depends on WHO is doing the reporting.

Family wouldn't necessarily know what would be in a nursing assessment or a care plan.

In that case, if it is the patient or their family giving information, then the healthcare professional should be able ask appropriate questions in order to elicit the relevant information... if that is what fangelam is asking. There are no four things that MUST be reported in every case. Each case is different
