Is it only me or are others seeing patients being put on Vancomycin as a first choice antibiotic? I've seen little old men with uti's, not septic, just a low grade fever and dehydration, put on Vancomycin right off the bat. Talk about creating the "superbig". And what are we doing to these people kidneys? Anyone else see this?

The broad spectrum abx we use before knowing what actual bug there might be, if any, is zosyn and we use kefzol peri-operatively.
If a person is allergic to penicillins, then we use vanco as a broad spectrum.
I personally haven't noticed an increase in the use of vanco overall.

I remember my clinical rotation in Jan of this year at a Hospital.........I freaked out at the amount of patients on vanco.......

Most of our Vanco patients are usu renal players with multiresistance....we haven't been using it more often I don't think...We use a lot of Tequin for UTI's...

We use a lot of Tequin, Zosyn, Rocephin, and Kefzol...not much Vanc though.

I haven't seen Vanco routinely used for UTI's unless the patient has allergies.
Pretty soon Vanco isn't going to be effective.

I see a lot of vanco being used, but its usually given for those with long standing history of MRSA infections..
Ive seen vent patient being treated with vanco for MRSA, off of the Vaco for a week, spike a temp and placed on vanco until cultures proved otherwise....You see this done more in the elderly in my opinion.

We started using Avelox for our MRSA patients..so we really don't use vanco like we used too...

I have seen this. Like the ER docs might put a pt with a UTI on Vanco and let the floor docs change the abx after C&S comes back. I really hope this is not a trend and I don't think it is. We need Vanco as long as it will work for us.
One I forgot about, we actually still use a lot of IV Cipro for our urosepsis...seems as though it fell by the wayside for a bit, but is back again

Use of certain antibiotics, Vancomycin included, is restricted in my hospital. It has to be approved by the Infectious Diseases doctor before pharmacy will dispense it.
