i know some of us have received our letters so i was wondering who else i will be making this journey with for the next 2 years.

my name is allison shaffer and it's nice to meet you. i know we will all be close because it's a small group. it would be nice to get to know some of you before november 17. :)

I'm working on my MCP, can someone put the order of DX for me?

I know it's ABCs then Maslow's, but what order of the Risk for etc.

What diagnoses do you have? I think the risk for dx should follow the same ABC and Maslow's rules.

  • ineffective protection related to inadequate calcium absorption as evidence by end-stage renal disease.
  • risk for impaired skin integrity related to stomal revision.
  • death anxiety related to perceived proximity of death as evidence by reoccurring ovarian cancer
  • risk for falls related to age as evidence by patient’s age of 75

I have never heard ineffective protection so I had to check my Ackley book. :lol2: I would put ineffective protection, risk for impaired skin integrity, risk for falls and death anxiety as last one because that is psych issue.

But:

I am not sure if you can put ESRD in your dx since we cannot use medical dx, right? The same with ovarian cancer. I usually put: r/t disease process instead of the med.dx.

If your instructor is big on proper nursing diagnoses then you need to put:

protection, ineffective

skin integrity, impaired, risk for

falls, risk for

anxiety, death

What do you think?

I think you would put 'Ineffective protection related to inadequate calcium absorption, secondary to End-Stage Renal Disease".

As far as ineffective protection, I've never heard of that one either...guess I'll look it up. (May use it sometime..haha)

(I usually don't put an AEB on the DX column, but I have seen others who do.) My only AEB is on the Goals section.

Yeah, I use the online Ackley Care Plan generator A LOT!!!!!! That is exactly where I found the ineffective protection, and I have never heard of it either, but hey- I'll use it!

Mrs. J. requires us to ALWAYS use a 3 part RN DX.

So she wants us to have ex:

alteration in comfort r/t surgical intervention as evidence by facial grimacing;

so maybe I should take out the actual medical dx- just in caes.

I hate doing nursing care plans! I understand why we need to know how to do them, because it all really ties your care for your specific pts problems all together! Its just so time consuming! and I hate paperwork!

As I remember I used my textbook to fill out the patho statement.

I also used Wong for C/S...but I sometimes use Wikepedia for online help sometimes.

I had my first ICU clinical yesterday. It was an amazing place. It was also scary. It was my first time seeing a pt intubated on a vent. I didn't know this pt but when the family members started crying I almost did the same thing. :uhoh21: Do ICU nurses cry a lot?

How many days do you get to stay on ICU,Katey, does everyone in 4th semester gets to do ICU? That must be so interesting. Our instructor will let us observe one day in the ER and ICU this semester.

I had my telephone interview with Charlton methodist for the externposition today, phew..........

They sure ask a lot of questions, I thought it will never end . I will have face to face interviews with the managers of L&D and postpartum next week. Wish me luck, I know they don't have as many position as they had in the summer. So I sure hope I will get one.
