Test to Stay forms and documentation

Missouri has instituted the Test to Stay option. Would any of you that are currently doing this option, be willing to share your forms? I would like to be able to streamline it the best way possible. 

I am thinking a blanket permission for use for the school day and sports. Just not sure of the wording.

Also, I think that a flow sheet of some sort so the documentation can be limited to one page for each student as far as keeping track of test results, dates, a place to document that I asked if they were having any symptoms. I'm sure there is more that I need on it, but not thought of yet.

Any help or direction would be great.

Here in MA, the documentation (including the consents) are all done online through the lab or vendor that has the contract for testing. We can also print out various reports including tracking.  Is that an option for you in Missouri?

Thank you for the information. This isn't an option for the Test to Stay.

I use the paper version of the CIC form (I'm also in MA).

For test result tracking, I built myself a Google spreadsheet. But as School Nurse Leader I also track also both our school buildings - the sheet is shared among my nursing team (there are four of us, two RNs, two LPNs). 

It basically has the following columns:

Name; Grade; Last date of exposure; Date notified of exposure; Day 1 Date; Day 1 Result; Day 2 Date; Day 2 Result; Day 3 Date; Day 3 Result; Day 4 Date; Day 4 Result; Day 5 Date; Day 5 Result; Day 6 Date; Day 6 Result; Day 7 Date; Day 7 Result; Day 8 Date (if needed due to weekend); Day 8 Result

(Test and stay runs through day 7 post exposure here in MA. I put in n/a for weekend days).
