Telemedicine NP work?

Looking to hear from those who have worked in telemedicine as a NP.

What was your rate of pay?

Describe your experience: likes and dislikes? Agencies you worked for? Etc.

For those who work in a restricted state, did you have to find your own collaborating physician?

1. ForeFront, Faspsych, and Genoa are a few of the larger ones.

2. Most will pay 1099, no benefits but good money.

3. Depending upon the state you may or may not need to be in that state.

4. There can be issues like prescribing a stimulant for a person, if you haven't done an initial in person assessment (unless the particular clinic where you work has a waiver).

These are just a few of the broader points.

My wife earns about 175k per year from home working 40 hours seeing patients and another 15 charting.

Now this is what I'd like to know as well:)

I believe that it is possible but there could be some issues:

a.  I believe that Medicare requires that providers be in the United States when services are provided including telemedicine.

b.  There is a currently suspended law due to covid called the Ryan Haight Act that requires an "in person" assessment at least for one visit in order to prescribe schedule II medications such as ADHD stimulants (this will also be an issue for me even living in Florida and seeing clients in Washington. Thus I expect to have to start going for one week per month to see initials once this law is put back in to effect). 

c. You would have to comply with local Visa and work requirements. 

d. Not sure how income taxes would work.

e.  If you were "cash" and or select insurance only it would be much easier.  

f.  Also certain US territories like Guam, America Samoa, USVI should also be okay even for Medicare.

According to another source, you have to be in the US if you accept medicare/medicaid. You can take cash pay but malpractice won't cover you. Only speaking for psych here, I don't know how telemed works for other specialties or how much it pays.
