With the aides I had the pleasure of working with on the floors, this couldn't possibly work. Some of them spent more time in the break room than with patients. Most of the LPNs I've worked with have been great, but still, I won't take responsibility for someone else.

I got written up once when a nurse associate (aide with one year left of BSN school) helped me transport a patient to a tele floor. First because the O2 tank ran dry - okay, that was my fault, I was new and didn't check it. But the big reason was that as I was settling the patient, the aide was behind me taking the nitro off the pump, leaving the nitro off and heading back to the unit with the pump. The patient said "I'm starting to have chest pain again" and I turned around and the pump and aide were both gone. My manager said "She shouldn't be touching the pump!" and I said "I know! She was behind me!" And a nursing student too, should have known better.

Ya know, team nursing can be great with all the right ingredients. I trained team approach and it worked well (70's-80's not the 50's) Facilities today are trying to reinvent the recipe unfortunately.

I worked a rehab hospital as agency that got it right and it was a great experience...I didn't mind the responsibility because I had a good LVN and CNA working with me.

The move to primary nursing and the elitism seen today has destroyed what used to be a great and affordable system of providing care, IMO. Why aren't there as many good CNA's anymore? Why are LVN's frustrated? Because their contributions are minimized today. Sad.

I don't understand why the hue and cry against team nursing.

They've gone to primary where I worked and the RN has ten to twelve patients on a busy M/S floor. They do meds, IVs, insulins, BEDBATHS.....preps for procedures.....and who the {bleep} knows what else.....on ten to twelve patients per nurse?

When I worked there we did team nursing. I ALWAYS had eleven patients, that was BAD ENOUGH, but AT THE VERY LEAST I had a CNA to do I & O's, transfers, baths, etc.

If we were VERY VERY LUCKY, we had an LPN to do the PO meds.

Even with someone doing PO meds and a CNA we worked our butts off....and this was on nights.

How anyone can want primary nursing is beyond my comprehension. All that is is an excuse to get rid of the RN's helpers and load the RN down with more work......I don't get it.

Well those 'nursing experts' who have pushed primary nursing have played a role in these type situations we see out there today, IMO. I was appalled to come to Tx from Minnesota and i enountered this exact scenario in my first medsurg job here....11 medsurg patients to manage totally on my own...it WAS unsafe on this 'progressive' primary care unit.

I feel what we need are more savvy nursing leaders who KNOW how to provide cost effective and safe care in a REAL and EFFECTIVE team environment...and welcome our LVN's and CNA's back into the healthcare fold.

We used team nursing in Louisville about 6 years ago and it was a failure. BUt like a good failure that promises to save money, management stuck with it for a long time. Even getting rid of CNA and house cleaning staff( Hold the code while I clean the commode!). Failed miserably when managment was left to care for patients as most nurses RN and LPN left. If you stayed and showed loyalty, you were abused. Team nursing is nonsence, staff reductions by any other name.
