What is the average time to start a pt. It takes me about 2 hrs and I use the oasis page by page. Others tell me they do it in an hour, others tell me they don't use the oasis in front of the pt, but rather fill it out after the visit.

Learning how to gather the information needed in a time effective manner is part of a skill set.

Long ago, when I worked in a high risk acute care setting, I learned how to conduct an admission/intake interview while providing urgent care to the patient. I had the multi-page assessment nearly memorized and simply asked the questions from memory and jotted down the answers as I provided my care. Since I went through the process in pretty much the same order each and every time it was easy to follow my notes.

Much later, when I began working in HH this skill benefitted me and allowed me to gather large bits of information WHILE I was completing the physical assessment, doing the dressing change, etc. I was in the habit of completing as much of that OASIS documentation before I moved onto the next patient, as I despise taking work home with me after an 8 hour day (especially when the employer does not want to pay for that time). This meant that yes, I did use the paper form or the laptop in the presence of the patient. The patient and family should be comfortable with the tools of your trade and the form or the laptop are just tools like your stethoscope. After the physical care was complete I sat with the patient/family and started in on the document itself, filling in the blanks with the information I had already collected. I was then able to address any areas that needed clarification or more detail and it made certain that I did not forget any required information.

When it seemed appropriate I left the home and attempted to complete that documentation in my car or nearby coffee shop when at all possible before moving on. You do get faster with practice and effort.

I really really hated the documentation in HH.

I do alot of SOC. At first it took me 3a hours visit and documentation. Now, most of them are one hour in the home with some documentation and one hour to finish documenting unless it's a vac, significant wound care or other direct pt care or hands on with teaching.
