Does anyone use SBAR for their shift-change report? If so, do you have a copy that you can share?
If not SBAR, do you have a sheet that you've devised to stay organized that you can post for downloading?

Ah, SBAR. Another thing they are trying to shove down our throats, saying it will make patient care safer. I won't use one. I've used my own little piece of paper with the information I need always written in the same places, so that I know where to look for what.
The form they want us to use is too big, and find stuff on it is not that easy. When I give report, if the oncoming nurse wants to use one, she fills it out as I give report.

We've gotten on the SBAR wagon too. I don't like having to change a report sheet, and will stick with the same one I've been using forever. It works for me and I don't see the need to fix what ain't broke. If you're giving good report, you're usually SBARing anyway. You just didn't call it that before.
My sheet is just a bunch of lines going horizontally and columns separating pt info, dx, dr, diet, act, I/O/IVF, VS frequency, misc info. I give each couplet (mom & baby) about 4 lines and that gives me room to add in extra stuff as I see fit.

I don't like SBAR for report. When I get report I don't want to know the situation first, I want background first. What is dx, why are they in the ICU.....that is background. Then tell me the situation. Assessment I don't even really care about - I will do my own assessment.
I do use an SBAR format for doctor calls if the doctor is not familiar with the patient.

we are forced to use SBAR and i hate it.. what a waste of time .. filling those papers out every shift .. our hospital has a approved form we must use... ugh..
We have an approved form too, but only about half of the staff uses them. So far, they haven't gone on a witch hunt to dump the rest of us in a cauldron. I suppose that's next. Never mind actually providing good patient care; pay more attention to papers which are approved.

I posted a thread about this and then found this one. We have a form and we use SBAR but hate it and as other's said it is way too much info.
I wouldn't mind sending you our form, but I'm looking for something better so I don't think you want ours..lol
Any advice from others using SBAR as to how to make it flow smoother and not be so lengthy would be wonderful!
