Has anyone applied for the program?

Why do you ask? Did something happen to your app because of it, or are you just curious? My best guess is for comparison to transcripts that have a former/maiden name. Maybe for background check as well.

I did not used my former name in USA. I eneterd in USA with my married name.

I did not fill former name on my online application because as I remember correctly ...It said fill out former name(s) used while studying in USA. I did not used my former name at all. I did all prereq at delta with my married name. None of my transcripts have my former name.

I am just wandering that if I still have to fill my former name on my document checklist packet. I am leaving it blank.

May be I am over thinking here....

Sounds like you are fine leaving it blank, but maybe call and ask, just to make sure. Better safe than sorry:)

I called already and got two different responses from two different people

One said ...Its my call,,,,I can leave it blank or fill it up... there is no record of former name .. ...

Other said.... I should not make any changes now.

On document checklist packet, former name is not under required info

I think delta is asking for social security number for background checks ...background checks are done by your SSN

Hi Everyone...I too, same as RNbound83, am a 1st semester Delta ADN student, If you read our thread from last semester you will find a ton of answers as we were a pretty close group, biting nails and pulling our hair through this whole process! I looked back and saw we were asked to submit docs around Oct 12 and didn't receive an answer until Dec 6! What a nightmare! Sorry I hope the torture is not that long for all of you! Good luck and my advice is to be ready with finances for books uniforms and other supplies as it is a big chunk (1200-1800 approx) to come up with in a short amount of time..we had to do it over Christmas which was pretty tough!

To all those currently applying, have you guys turned in your documentation yet? I am delivering mine tomorrow in person. Excited to finally have this application out of my hands for good!

Another note -

As it turns out, you do not have to submit documentation for the life circumstances, which you all figured out. But dummy me, I was super confused because when I talked to one of the people from the nursing staff, they did not correct me when I stated that I would be submitting my BOGG fee waiver stuff. Moreover, from reading past posts from earlier threads, at least one student was disqualified for not having the proper documentation in this area.

Apparently, this is the first semester where they are only requiring the attestation sheet only. My guess is that only those who are borderline accepted will have to submit those documents later. But eh, who knows.

Hope I didn't confuse anybody!

BTW, does anyone know when SJDC puts out their fall schedule? I'm curious as to what time nursing classes take place and on what days. I have already looked through the archives for clues.
