There are NO CMS requirements that a copy of the 485 be in the patients home. Better have a copy of the med sheet though. Maybe your agency uses the 485 for that purpose.
Med sheet must be in format PATIENT can understand, no BID or TID abbreviations.....state, JCAHO and MEdicare surveyors have been looking for form on tracer review.

Can you clairy term "PDN" is that private duty nursing?Home health aide private pay patients DO NOT need signed physician orders unless required by state regs. They DO NEED HHA care plan with written policy on frequency of review. Ours reviewed every 90 days.Spot check HHA notes to make sure match care plan EXACTLY to make sure followed and avoid getting dinged by state.Advise your agency subscribing to "Home Health Line", industry newsletter as they had great example last month of such forms and provide info re regs requirements.

If anyone got that I was riduculing the newly appointed DON, I'm truly sorry. I really was trying to be helpful for her, and shared the methods used in HH agencies where I have worked. They were in 2 different states, VA and CA. and there was no difference in the way the forms were used. It seems that some of the anxiety I ws feeling about my personal situation must have leaked between the lines I wrote. I've read and re-read what I wrote last August, and don't know where accusations of "ridicule" are coming from.
I tend to "call {things} as I see them", and without seeing my face, that might be thought to be critical, but ridicule? I shared my experience, gave information and concern genuinely. Perhaps too directly..... Things in my life were rapidly falling into the chasm called mortgage fraud (committed by the bank). I was hurting terribly. Yet the opportunity to be useful was attractive, and I dove in, obviously into unfriendly waters....

I would have the same question? Would like to know how long Ms.RN'02 worked as a field nurse? Does they agency know that it is a requirement in the Medicare regs that the DON have at least 2 years field experience in home health (I could be wrong, but only on the 2 year part, may be 1 year, but it is specifically stated in the regs).??

Hello ladies I am not a nurse I am a biller and the form (485) you are looking for can be found if you download the PCACE Pro32 software from medicare. They have the plan of there and its free. I hope this help.
