Question on how to put my name on application for board licenseure

On the application for the license, it asks me to put my name obviously. But my transcripts and everything will show my maiden name and not my current last name.

I came from the Philippines to the states right after I graduated and have now just got my diploma after getting married in the states.

Should I put my current last name on the application or my maiden name? Since I can't put my married name on the transcript request form because I wasn't married when I graduated and my transcripts show my maiden name.

Please help.

Also.. I don't have license in my own country in Philippines.. will CA board still accept a written explanation as to why I didn't have license there?

I would put your married name. Just place a copy of your marriage license with your application.

But what about for my transcript request to my school? When I graduated I was still in my maiden name so basically my school has my maiden name only. When I submit my transcript request to my school, should I put my married name or my maiden name?

Hi. I too have graduated from the Phils and gotten married when I came back to Cali. I'm not sure if we're in the same situation, but this is what I did. When I applied, I simply used my maiden name all throughout. I decided to have my name changed to my married name as soon as I passed my board, considering how difficult it could get to have your name changed in the school you attended to (well, at least for my school, that is). Not to mention if you were to do that while you're here in the states. Well, that's how it worked for me. Changed my DL, SS, and everything else as soon as I passed my board to avoid any mishaps or additional required papers to go about with the processing. Just the same, I suggest that you call the BON just to make sure which would be better. Goodluck to you.

You are aware that Ca have been getting strict on people obtaining local licenses with exception of not being able to sit the exam as they are not a Philippine citizen? You may find submitting a letter may not be enough.

I hve moved your thread to the International Forum as you have additional requirements that need to be met for licensure in CA.

First, not sure if you are aware of the fact that CA is no longer permitting someone to sit for the exam if they do not hold a local license. Suspect that you came over on a K-1 visa, this means that you are still a citizen of the Philippines and are not exempt from writing the NLE since the PRC will permit you to do so.

That you came to the US before you had a chance to write it is not going be an acceptable excuse to them. Much has been documented on this and it has actually been in effect since the beginning of this year. Even dual citizens are not exempt; only those that are not legally permitted to write the exam such as holding citizenship only in the US. But this still also requires an actual letter from the PRC that they cannot sit for it per law there. This would not apply to you.

Listen to what HeartyRN is telling you. Always best to keep everything in the same records that they were in, for you this would be your maiden name, even your interview at the US Embassy was in this name.

Then afterwards, you can submit a copy of the marriage license and then things can get updated easily. Anytime that you make changes to names in the middle of processing things can get lost. And even more important is when the records are coming from out of the country.

I'd also like to go through a state that does not require language text like tofel etc. That is why I wanted to go through CA but seeing now that I can't use them without a local license, which state should I go through?

Also, I can't apply in my maiden name for the state boards because my name is already changed to my married name legally. Could I just submit the transcript request form to the school in my Maiden name but submit my state board application in my married name along with a marriage cert to show why there is a discrepancy between the transcript form and my application?

What state are you living in? Even if you go to another state to gain a license you will in most cases have to meet the BON requirements for foreign trained nurse so not always a guarantee that they will not ask for your local license.

Requirements are normally the same for initial licensure as well as endorsement. Most of the states in the US actually do not have language requirements for licensure in the first place.

I have my Philippines Passport and the Visa I came here with in my Maiden name. However I have a US drivers license, social security card, and green card in my married name..

So should I apply everything in terms of my nursing license in my maiden name and submit the forms along with my marriage certificate?

Also,

I am currently in the states. How can I take my local board exams? I would have to go back to the Philippines and then take the test there? That isn't possible for me. SO I guess if every state requires the local license then I have to give up on my nursing career?
