Pyxis problems

Published

When wasting narcotics we need another nurse to witness the waste with in the Pyxis. Here is my problem, have any of you ever went to waste a narcotic and the Pyxis DIDN'T ask for a witness? It has happened to me a few times. Now I am being brought in front of the BRN for investigation regarding this issue. I have contacted the Pyxis manufacturer but still have not heard back from them. It would help me if any of you could post if this has ever happened to you. The hospital that I worked at basically called me a liar and terminated me for this.

yes, I understand that even if the Pyxis didn't ask for a waste that I have to document it with a witness. I did document with a witness and when I told the manager about the Pyxis not asking for a witness she basically called me a liar "because nobody else reported that issue". My ORIGINAL question was "HAS ANYONE ELSE HAD THE ISSUE OF THE PYXIS NOT ASKING FOR A WITNESS?"  With all due respect, I don't really care if you don't use Pyxis, glad you don't use Pyxis, or you use Omnicell. I am looking for answers to my question. I don't enjoy being called a liar when the Pyxis DIDN'T ask for a witness.

People here are not against you--it's just that your initial post didn't say anything about actually having done a witnessed waste. I'm sorry for what you are going through, it sounds beyond unfair.

Unfortunately this sounds like a situation that might be highly specific to your hospital -- their policies, their programming and maintenance of the Pyxis and especially their attitude toward nurses.

Yes I have had Pyxis not ask for a waste in situations where I wanted to note that I wasted the medication with a witness. I did what you did, which was note it on the EMR. No one came after me. Still, I subsequently decided that any hint of aberrancy with Pyxis would be resolved immediately with pharmacist and/or house supervisor, due to the downright hateful attitudes I have seen and heard about regarding these topics.

What did the Board of Nursing find you guilty of?

I find this very odd.  Because yes, there are times the Pyxis doesn’t ask for a waste, as in my scenario stated above.  For the BON to find you guilty, something more had to have happened.  You would not have been terminated for reporting Pyxis didn’t ask for a waste.  Especially if you provided one.

Nothing more happened and that's exactly why I'm suing the hospital,  the manager,  and the DON.

Has this been made a matter of "diversion" or some sort of false documentation thing or what kind of wrongdoing did they make this out to be?

They're saying I diverted 0.5 tab Norco. Even the Pyxis showed I wasted it and it would not let a witness cosign. The manager was out to terminate me ever since I married my husband. That and I hated trump and she was a diehard trumptard. I'm not just saying these things. I have signed affidavits attesting to this.

How about the person who witnessed your waste?

Always--- always--- notify your nurse manager and pharmacy for any issues with Pyxis. I have seen people accused of stealing drugs when all t's were not crossed and I's dotted!

Normally the witness when wasting setting is set in the Pyxis server. Someone from pharmacy should have checked to make sure the settings were correct for that medication.
