If you have a valid license in any American state, yes! If not, NO I assume you're in Phil applying fresh, so NO. Also, form 3 isn't needed!

Oh no, I bet you can call CGFNS and inquire. Because I did that very often 'cause my college claimed they had never received any documents but in fact they did but it was laying in the registrars office. So make sure you do call them and ask and also...
