Hi, I just applied via breeze for PHN INITIAL application. I just paid and I noticed that it did not say anything about getting your transcript from your school. Do I need to do that? Anyone who applied thru breeze for phn? Thank you.

Kudos! I myself is a clinic nurse for elderly people. Mostly it is case management, but I like it. Plan to move on becoming a PHN in the future.

CGFNS needs an updated point person of your school to have the signature. Maybe your school has a new person assigned or signed the form you gave them. Thats what happened in my case and CGFNS sent a document to my school to update their point person...
