    Why should the CVP never be higher than the PAD? We do not wedge our patients, but use the PAD instead to approximate the PAWP. If the PAD is lower than the CVP what may be the cause, and how do you troubleshoot it? Thanks! Newer CVICU RN.

    Alright, that rationale makes a lot of sense! Thanks for the reply. The reason we don't wedge, as a regular practice, is because the PAD closely approximates the PCWP, and there is a risk for the patient in wedging.

    Also, arteries are end-points, and meds are more likely to sit there, locally, causing ischemia, before they are finally taken back up through capillaries and distributed centrally where they are needed.

    Arteries are anatomically different than veins, and have a layer of muscle. They are more likely to spasm when drugs are administered into them, which can cause tissue necrosis.
