    NGT Inserted Into Lung

Yep several times over the years - pts also survived. You were watching and aware of what was happening - you did well.

    Misunderstanding in ER

I worked in a level one ED for 10 years. Agree there is probably more to this then meets the eye. If you went to ED for pain and they gave you narcotics, you probably shouldn't have been driving anyway?? Not sure what their complaint was except ...

New grad to NP. Is it a mistake?

I don't think its a mistake. However, I do think you need to realize that you will be seeing pts every 15 minutes everyday! Its grueling and although the knowledge base will be provided in your cardiology fellowship, the paperwork and quick pace wil...

    Floor RN transitioning to dialysis

Agree with above. Your assessment skills will be very important - the ability to tell sick from not sick. Best wishes.

Value-Based Healthcare and the Role of Nurse Practitioners

Our practice utilizes a value based care approach. I'm in nephrology so my pts are very ill. We are provided with coding instruction so that we can capture charges for the care we already provide.

CNS, an APRN role on life support?

Like everything in nursing, the states run the system. I'm in IL where an APRN is an NP, CNS, CNM, CRNA. We all have the same practice act and we all bill. As a CNS (adult and peds) if I couldn't bill just like my NP colleagues I would be out of a jo...

    Ethical Question

I don't think it was unethical. I'm thinking this is discussion between friends ie PA and RN? Moral of the story - don't give out free advice. I will say as someone who is raising a child with ADHD there are so many shortages of meds at the mo...
  9. Nurse Beth, as always spot on! Thank you. I will say that as APRN I'm appreciated. However, I'm not naive enough to think this is personal. Nope - I generate income so I'm valuable....at the moment. I'm valuable kinda like a cell phone, couch, ...
  10. I live in a fairly rural area so driving is my life - LOL. However, if you live in urban area, driving between units wouldn't be a big deal.
  11. I'm in nephrology. I see pts in three dialysis units 4 days/week and then I have office (pts every 20 minutes) one day per week. Lots of flexibility because dialysis pts dialyze 3-4 hours. I prefer to go in early (like 5:30am) and then get off around...

    Post-master's certificate

I graduated with an MSN in 2005 in Management and Leadership. I then did an adult CNS where I had to take the three Ps and 560 hours of clinical. Then in 2011 I completed a peds CNS where I had to retake the 3ps with a peds focus and another 560 hour...

Fired. How To Get Over It.

I committed a med error that resulted in pt harm, was sued, successfully and have a judgement against me from a malpractice settlement. That occurred in 2007. I still am apprehensive every single time I prescribe this particular med. I ALWAYS look it...

Quit First NP Job Without Notice

My understanding was this was a hospitalist position. I think it would have looked better to not have quit during this time. Inpt is very busy and I get they were short-staffed. however, if you were still employed, it would look better.
  15. Nurses have bad days just like everyone. What's the difference between burnout and moral residue? This is an important issue for all nurses. Many people have thoughts about how to combat nursing burnout. There have been studies, conversations,...

Clinic NPs, how many admin hours do you have?

Hunh??? Admin time, what's that? I see pts in the office one day per week and the other 4 days I round on dialysis pts. I prep my office charts at home usually

How do you get the experience for NP program?

I had 10 years ED nursing in a level 1 trauma center plus a couple of years ICU exp prior to APRN school. Experience is never wasted. You don't need to know all the meds you are going to prescribe as a provider.

    Absences - Insomnia

As you are in treatment for this, perhaps that could be communicated in a general way to your boss? I wouldn't volunteer any info about your mental health. As others have said, its a small career field. I would like to say we are progressive when it...

Stuck in an NP Speciality Role

I was an experienced RN when I went to grad school: 10 years in a level 1 trauma ED, 2 years in ICU, 2 years in LTC. I interviewed with several practices/hospitals and got a nephrology job. Now, I had basically no dialysis or nephrology experience. H...
  23. I went to University of Phoenix for both my BSN and MSN - for me, it worked well. Yes, it was a for-profit school. I had no issue securing preceptors but I had contacts in the community already. Nursing is a second career for me - I was a milit...

Hospitals charging nurses $100/month for parking?

Okay and I whine when I have to pay $0.25 to park. I work in rural areas so there is plenty of parking even if I have to park in the cornfield.
