I got the surface antibody. And I messaged the administrator assistant and she said we will find something that says "declination signature & date" and that is what we are supposed to sigh on our health form.

Hey! I scheduled an appointment with my doctor and I just got blood drawn and actually received my results for the titer today. I would just schedule an appointment with your doc and show them your paper work and everything you need.
