I'm a big fan of Hemodynamics made Incredibly Visual. It seems like a fairly basic book because it has a lot of illustrations but it's an excellent resource especially when you're first learning about hemodynamics.

I agree - keep them 1:1 until after extubation. Typically the first big hurdle is to get the patient extubated. If they are very sick and unstable - they're usually going to remain intubated. Maybe the compromise could be that the patient is paire...
