I bet this was a test to analyze whether the person was trembling and evidently nervous, or if they calmly could handle the task without appearing to be stressed out.

The key is assessing your patients early, quickly and efficiently & charting the asess.; priortizing; getting your meds passed early (1/2hr) or on schedule to stay atop of things; and responding to problems, phone calls, pt/family questions/teach...

a No-No! Only chart what you have actually assessed. Plus, it seems like you are making more work for yourself. You can't actually chart a presumed assessment. tsk tsk
