  1. Duramorph and foley removal...

Oh wow, where are you and how is that going? What are you doing instead?
  2. PTO accrual

Hi everyone, I am considering a new position and the PTO accrual for the first year of employment is next to nothing. Would you mind sharing with me the percentage that your facility uses for the first year of employment? Thanks!
  3. PTO accrual

Thanks everyone for your input! Year one earns 1.5hrs per pay period (72 hours). Year 2-4 earns 4.5 hrs per pay period. Blah!
  4. when to call for delivery, active management & patience

I'm interested to see your answers. I agree with #1, it is very hard to predict and time just right. But it is there job to be there. I worked nights. Sounds like you work days, which be a little more difficult to coordinate. I think #4 isn't a bad i...
  5. "Short Staffed......."

lindarn, Excellent post! So very true. Not to take away from teachers and the struggles that they face, but we are talking lives (mother and baby) in our hands. My husband was asking me the other day to explain nursing staffing to him to L&D. I t...
  6. OB Antepartum Care Plan

Whenever anyone falls we always draw "abruption labs". CBC, Fibrin Split Products, Fibrinogen, ABO-RH and a Kleihauer-Betke Stain. We generally observe mom and baby and repeat the blood work in 6-12 hours depending on how often they order it.
  7. once again... about effacement

    I think it would hurt!
  8. possible to be ruptured and be closed?

I think the OP is referring to rupture of membranes....those are my least favorite to admit. Ruptured and closed...and armed with a birthplan. Always feel so bad for them!
  9. pre-delivery "grooming" issue?

I've never had a patient tell me that their doctor told them to, but I bet they are influenced by what friends or family tell them. If the subject comes up, I tell them that we definitely do not shave anyone. Except if they need a c/s, we'll shave ne...
  10. pre-delivery "grooming" issue?

Perhaps I am the lone waxer here. I agree with the others that it is not right to not do it if it is an expectation, but I do it because I prefer it. The reason I wax as opposed to shaving is because when it grows back it does not itch! No razor burn...
  11. pre-delivery "grooming" issue?

I could not wait to log onto this thread tonight to see the direction it had went....never could have guessed......HILARIOUS :hpygrp::hpygrp:
  12. hotel stays

Anyone here ever stay in a hotel during the the day between 7p-7a shifts? Are the rates lower than at night? I am looking into a contract with a 2 hour commute and this would be the only way I'd consider to do it. Thanks!
  13. help w/ cervical effacement

I agree with klone, it is very, very subjective. I usually err on the the conservative side and reserve 100% to paper-thin. As long as you tell if there has been change or no change is what is really important!
  14. Recovery for vaginal delivery

I don't know how you'd ever get all of that done within 2 hours after placenta and you didn't even mention all of the charting and teaching involved!
  15. Vag Exams & Dilation

When the patient is 8 or more, I feel around the whole head and estimate how much is left of the cervix. So one fingertip=9cm, almost 2 fingertips=8 cm. It is also not uncommon for the cervix to be complete on one side and 8 or 9 cm on the other. In ...
  16. Hi all, My 2 year old cat had a urinary obstruction this weekend. He was in the hospital until Monday afternoon with a catheter and IV fluids. I worked Monday night but he had supervision at home. He still is pretty under the weather. So I called off...
  17. Evidence-Based practice re: Pitocin Inductions

Thanks for that link. I thought this part of the drug instructions was interesting "Once the desired frequency of contractions has been reached and labor has progressed to 5-6cm dilation, the dose may be decreased by similar increments 1-2mu/min eve...
  18. post c-section pain control

I'm interested in what other places are doing. The CRNA or anesthesiologist gives Astromorph into the epidural space. Then our epidurals are pulled immediately post-op. We have standing orders for hydrocodone 5/325 1-2 tabs and also Motrin 600 mg i...
  19. post c-section pain control

Yea, just as I suspected. Our pain control stinks. Like I stated before, Astromorph in OR then Norco or Motrin. I had a lady with general last night. She had a morphine PCA however after 13mg, she was still in considerable amount of pain. We then swi...
  20. post c-section pain control

See I wish we were more generous with IV pain medicine standing orders. I really like Toradol. Are there any diagnoses or reasons that it can't be given post-partum? I'm assuming patients with abnormal coags would be a case. It's very interesting to ...
  21. Newborn Transition Policy

I am very new to L&D but I'll tell you how it is done where I work. Each shift we have a designated float nurse that will be present at deliveries and observe the initial transition of the baby. Either they or myself will assign the apgars. I try...
  22. Requesting Info about Transplant Nursing

Transplant nursing was my first job too. I did it for 3 years. I interviewed for a few positions in med-surg and it seemed the most interesting. It is certainly a specialty, but you will see many different diagnoses and complications. So it is very w...
  23. deep tendon reflexes

Does anyone have any tips on checking reflexes? For me it is usually hit or miss (mostly miss!). Do you use your stethoscope or carry a reflex hammer? Do you check the patellar or the brachial? Also out of curiosity do you check reflexes every hour ...
  24. Nurse tracking devices

We have the same thing and I really like it. It saves a lot of time looking for people and decreases overhead announcements. I have been using them for 2 years and have never heard of anyone being tracked on where they were and for how long. We use t...
  25. How many hours would you work?

I would work 12-24 hours per week. That 3rd 12 just seem like too much some weeks. 8 hour shifts seem so short when you are used to a 12. My ultimate goal is to be able to work PRN "when I grow up".
