  1. Managing symptoms for a good death

Thanks zenman, interesting philosophy! May you recommend some 'further reading' on Shaminism and more particular "death rites". As a Hospice Nurse, I have developed my own philosophy, and "spiritual practice", and also believe that "the body knows h...

  4. inpatient hospice facility question

Hi all, I'm contemplating working in-patient as opposed to out-patient case management. I've been advised that working in-patient may entail up to an 8 patient load with a CNA, and acuity will not factor in. Pros for in-patient include (1)less trav...

  10. Fentanyl Patch

Not to be disagreeable, but not wanting anyone to be misinformed. You may wish to check with yor pharmacist/MD for equivalency of morphine with fentenyl 12 mcg. Best wishes!
  11. Fentanyl Patch

Yes, I too have seen this reaction to a Fentenyl patch. I suggest you stop the patch and see if the patient perks up. The 12 mcg patch is equivalent to approx. 40-55 mg of morphine/day. Best wishes!

  19. Help Me Make Peace With Myself

Interesting topic, just another observation that I have concerning the use of morphine. I have witnessed family/staff giving morphine very liberally and the only apparent effect was increased secretions requiring frequent suctioning and aggressive se...
  20. Help Me Make Peace With Myself

Dear handyrn Excellent question and concern! My humble opinion is that the morphine may hasten the process (does anyone really know for sure). I usually try to maintain an actively dying patient with oxygen up to 4 lpm and Ativan on a q6 schedule, an...

  22. Managing symptoms for a good death

Michael, intersting points you make. Ego could be the social mask that gets stipped away at the end; Ego served it's purpose on earth, for survival, but does not have a place after our time here; those that struggle with their death, don't want to ...
