  1. Weed and work.

I would talk to your instructor. If he's to the point that he has an addiction, he's dangerous. It would be horrible he he hurt or killed someone because he was impaired or unable to concentrate because he hasn't been able to smoke.
  2. Lordy...6 admits in 5 hours....

I think those are the hardest kids to take care of. Sometimes nothing you do comforts them. We usually use methadone and or morphine to wean.
  3. NICU palliative care

What we do depends on the situation. If it is a kiddo that can go home without ventilation, We send them home on hospice. They can handle a nasal cannula, tube feeds if the family wishes. We usually leave the infant with the parents if it is a lethal...
  4. How does your unit secure IVs? (and feeding tubes, cannulas, etc)

We always use tegaderm over the insertion site. Then we use silk tape to secure the hub. We try to avoid covering the insertion site. I personally don't like arm boards, I have lost more IV's because of them than without. But our arm boards are cott...
  5. nasal intubation

Do any of your units nasally intubate?
  6. Ett suction preterms

Thanks NeoNurse I was tired when I got off work last night. Patency makes more sense than placement!
  7. Ett suction preterms

How exactly does suctioning verify ETT placement? You can be down the right stem, and still sution seretions out, plus usually if you are right stemed or too high breath sounds and usually sats are not as good as they should or can be. I can understa...
  8. Octuplets

She HAD to have used fertility treatments of some kind. I agree with previous posts: what dr in his right mind would do this?? Or even what parent?!?!?! We had a set of 23 week triplets born, and only one made it? I would love to have a 31 weeker wh...
  9. rotating shifts?!

Wow. How high is the turnover? 20 years of rotating? wow.
  10. Changing to Night shift

Hey everyone. I'm moving in the next few weeks to start a new NICU job. I have been at my first one for 1.5 years, and have been days since I started. I will be on nights at my new job. Any suggestions on making the transition from days to nights? ...
  11. Changing to Night shift

luckly I'm not rotating. I had to train all my friends not to call me at odd hours of the night when I got my dayshift job. I'm just glad that I'm having to rotate.
  12. rotating shifts?!

Rotating shifts sounds dangerous, just all the changes in your sleep schedule. I think most hospitals do 12 hrs, our unit is phasing out 8 hr shifts.
  13. Possible Phone Interview

I had my phone interview (today actually!) and it was much less structured than the phone interview I did with the recruiter. The questions were basically the same as a face to face interview. I find them a little less stressful because you can sit i...
  14. tattoos and IV's

With the exception of lab draws, the needle does not stay in. It is removed, and only the catheter is left. Even with lab draws, the needle is only in for a minute or two at the most the it too is removed.
  15. Kangaroo Care

It may have been nice to look at, but what did mom think? I would not be happy if my bf/husband/significant other did that. But on the other hand, being female, it still would be nice to look at.......:innerconf
  16. Platelet Transfusion

We filter all blood products. We have a separate kind of filter or the blood and another one for the platelets.
  17. Kangaroo Care

eww. But at least they have a fur rug to lay on!
  18. How do I explain my new job?

What we do is for their benefit, just as any nurse does for their patient no matter if they are 1lb or 500lbs. Luckly ours dont remember what happens to them. I dont look forward to going to work to admit a 23 weeker or start an IV on a kid with no v...
  19. How do you sanitize your hands?

We do a 3min scrub in at the beginning of the shift with our soap. Then hand washing or the EVIL foam. We did have a gel until recently, but it went away (sniff..) Ironically our infection rate has sky rocketed lately. But it could also be due to the...
  20. Kangaroo Care

hikernurse, your unit does not kangaroo with a high flow nasal cannula? Or are you referring to something else? I encourage kangaroo care with all my parents. Esp my younger ones when they are stable. I'm not fond of KC when they are vented, extubat...
  21. Talking to your neonate patients

I absolutely talk to my babies. Most of the parents like it. I talk to calm them, esp my bigger vent kids. I also encourage parents to talk to them.
  22. NICU Pet Peeves

Nobody should be knocking, tapping, or writing on the isoletts. There is nothing to absorb the sound. The decible level is high and can cause hearing damage.
  23. Bedside Supplies

We have carts at each bedside, which are restocked daily. We only empty them if the kid comes out of isolation. We usually move the carts with the kids, unless they go to our level 1 unit, and we have different kinds of carts in there.
  24. Ethical question

We have one like this right now. Parents felt guilty, and kept having us resusitate. Now he is comfort care only and they have said that they are not coming in. It hurts to think that this little one will have on body holding him, nobody (except us) ...
  25. They are trying to force the DTaP vaccine on me.

I'm in a NICU, and we had a mom give her child the Flu. I'm sure she didn't think she was either.... He was on the vent for 3 weeks because of it.
