Hi....I'm a first semester student in nursing school.....can someone help me do 3 nandas for diarrhea? My patient had a bowel resection surgery and she has incontinent diarrhea.....she doesn't consume enough wanter and loves to drink coffee and tea...she is 80 years old and already had a TIA (transient inschemic attack)......I can never figure the nandas out.....can someone please help me......I would really appreciate it...thanx

Hello, do you have a nanda book? Carpenito has a good one, showing the goals, interventions, etc for each nsg dx.

hi richard...i do have gulanick care plan book and sparks and taylor but my instructor doesn't want me to use anything from there....i kind of know how to write nanda and fidn them but to find the correct nanda for my patient.........thanx for your help though.

Why doesn't your instructor want you to use the care plans books? A.) That's what they're there for and B.) if you cite your information, why is it a problem?

What are you having trouble with in relation to your nursing diagnoses? If she's 80 and has had a TIA, it seems she might be at risk for falls. If she's got diarrhea and isn't consuming enough water, she's either at risk or potentially qualifies for a diagnosis related to dehydration. Hope this helps!

You need to provide signs and symptoms. Can't build nsdxs based on medical diagnoses, have to be able to work around the patient as she presents. I.e., skin turgor, good, fair or poor?, labs such as BUN elevated? etc. I'm thinking along the lines of fluid volume deficit. Plus after surgery, there's always risk for infection.

your intructor doesn't want you to use any nanda book or the specific one you have?

as another poster pointed out... that's ridiculous; the books exist specifically for students and practicing nurses to create these careplans. at the very least, you'll need a list of nursing diagnoses with which you relate the client's signs and symptoms.

good luck with this. i'm sorry you can't use a textbook.

my instructor said the care plan books are too weak ats why can't use them.............i figured out the nandas already so thank you everyone...the problem is dat i have 2 much paperwork and have to write everything not type.....so dun have enough time to write every information out here............and daytonite i did have all my assessment done including the labs, skin, and everything else............it's just that i have so much goin on and um trying to pass..........next time i'll just post nandas when i get done with the as evidenced by so it is easier for everyone to undertsand what i am talkin about......ps i just started nursing school and this is only my 1st careplan.....im still learning so if i make ome people mad by asking them questions then um sry but i thought this forum is to help people understand.................

just curious, what resources are you allowed to use? i know for my care plans, all interventions and outcomes have to be cited because our instructors don't expect us to be able to come up with these things on our own. what do you still need help with? i know i'm not mad that you've asked questions here, it just seems like you're asking someone to do your homework for you, which is not something i think anyone here really has the time for, not to mention that it's imperative to learn some of this stuff on your own.
