i'm new here and figured i would try and see if i could get some assistance. this will be my first care plan to do and i'm very nervous. i did pick my pt this morning and have been stumped since with trying to come up with a nursing dx. here is what i have so far on my pt: 85 yr female, dx constipation, anxiety, depression, hyperlipdemia and uncomplicated demetia. her lipid panel is now normal. she's in a wc and wanders around. i am thinking of going with constipation but i dont know. can you help me with guiding me to the right direction? thanks all!

You could go with constipation or risk for falls.
There are a lot of things.
I usually look through NANDA diagnoses related to my patient's body system issues and pick one that I can find a RT and AMB. Then you just need to think of the desired outcomes, interventions, and scientific rationales.
We did 3 care plans this semester.
I'm getting the hang of it.

Do you think that your patient is a safety risk because she has dementia and wonders around? If the answer is yes, I would make safety my priority diagnosis; safety first. If not, constipation is a good Dx.
