Nurses Notes plagiarism at work such a thing?

Any one ever been accused of plagiarizing another nurses nursing notes. I don't mean whole paragraphs, just a few phrases here and there, like "ambu bag at bedside" or "I & O cathed x 1 for clear,yellow urine" or trach care performed. Pt. tolerated well". In my state SC the SBON does not have anything regarding same. Just want to know what is the general census regarding this.

I'm a pre nursing student and I have problem shortening phrases in the past. Throughout my academic experience I have learned that there are people have better notes than I have been. I don't necessarily steal their not3s. I do ask how they do it. However, I'm better at creating notes. In my world, I don't see it as plagiarism. Citing notes at hospitals or health care facilities would be a disaster in workplaces.

As a provider, when I see pts in the hospital, the way we do our charting is to copy and paste the previous providers note (the provider from our practice). Then, we change what is necessary and save it.

Not plagiarism at all - just ensuring all bases, diagnoses are covered and that the plan of care continues.

For goodness sake, this isn't expository or creative writing here, it's just a description of the patient's status and condition and what's occurred.

The term plagiarism has no application here.

Joint commission and other accreditation bodies look for key phrases when they look at documentation. So I don't see how it's plagiarism Obviously those bodies consider it ebp to use certain phrases in documentation.

I don't think evidence based practice is what is going on here. It's just common practice.

And, no. It is not plagiarism. People need to understand what plagiarism really is before applying the concept. Unfortunately, not even educators are using it correctly... Sigh.

For instance did they copy that a patient went for test that the patient went for on another shift? That could be false documentation.

Yes. False documentation is another issue. Probably what was really implied in this scenario, right?

I do have a pet peeve though. You know in EMRs when a provider will just cut and paste the previous note and add whatever happened on the new day? Common for consult notes, general progress notes from attendings. Ok, couldn't they still proofread? I have seen erroneous information copied over for days and weeks even! If I am cutting and pasting and the new note now has my name attached to it, you bet I'd be making sure I agree with everything on it. It's funny sometimes, but really not as this is a part of the permanent record.

LOLOLOL....I know right. I have seen some CRAZY cut and pastes and embarrassed providers.
