NP job paid by dictation notes not by hour

Published

I got a part-time job offer working at a LTACH. The physician offers to pay me by the numbers of dictation notes ( H&P and progress notes) not hourly salary. He offers to pay me $30 per H&P per patient, $20 per progress note per patient. What do you think? Please advise, thank you

Not sure what you are asking. What billing codes will you be submitting? How long does it take you to do a H&P and progress note?

Sounds very, very low. 30 dollars for an intake that may take you close to an hour? 20 dollars for a visit that may be complex and take you a half an hour?

Nope.

The physician is doing billing code with Medicare, I am not sure. It takes him 1/2 for H&P and 10 min for progress note. Time spent to interact with patient, chart review, coordinating care.....does not count, thanks

It is important to know how much your services are being reimbursed to establish your value. A simple way to negotiate is take your expected time and calculate into your required hourly rate.

It is unlikely that it takes him 10 minutes to complete a progress note or 30 minutes to complete a H&P with regard to charting only. I have used many electronic, dictation and paper charting systems and the actual time even with a cumbersome application is only a few minutes for progress notes and maybe 10 mins at the most for H&P. The 10/30 probably does include his assessment of the patient.

This is very helpful, thanks!

Your pay would probably average out to 50 dollars an hour, quite low if no benefits are included.

I ain't doing piecework. Especially not at a discount

Jobs that pay by the patient, not by the hour, can actually work out in your favor.

My experience only.

Yeah I've actually seen that with some of my NP friends who do behavioral health in private practice. However, they are guaranteed a base salary and get performance bonuses for seeing extra patients. I haven't seen any get paid just by the patient & this guy seems like he is looking for a bargain. Red flags abound here I'd say stay clear

I have a per encounter w2 job, no guarantees, and never make less than $125 an hour.

OP everything is negotiable, it is reasonable that an employer will try to get you as low as possible. Make a counter.
