It doesn't look like they're available online. There is a place on their website that you can request that they mail you one.

It does not explicitly say they will. The form is for US addresses.

thanks, I already emailed them yesterday, however still dont have a reply from them.

one more question, with regards to the fingerprinting card. Do I request this too? How can an international student accomplish this part when all locations for fingerprinting are in nevada?

Do I have to accomplish before taking the NCLEX?

if you do a search in the Phillipine forum I am sure there was a thread answering a few questions regarding finger prints and I am sure it has to be done before they will consider your application for licensure and recieve the OK to sit NCLEX
