I have trouble answering these types of questions. I feel like I only get 50-60% right. You guys have any suggestions for answering these questions. Thank You!

When it comes to that type of question, I have always relied on my gut.

Usually open-ended responses to things a patient says or repeating or paraphrasing what they say back to them as opposed to yes/no questions.
In all your prep material you can probably design a test (I know you can w/ Kaplan Q-bank) that will ask you 50 psychosocial integrity questions - maybe do some of those. I just know even from nursing school the subject I freak out about and worry that I will not know answers to on the test is usually the smallest portion of the test! I mean, everyone gets some answers on the NCLEX wrong - so don't stress - know your norms, know your fundamentals!

Also, asking a patient a "Why?" question, a yes/no question or saying things like "there, there, it's going to be alright" are wrong nearly 100% of the time.

NO why questions, NO questions that can be answered with yes/no, NO passing the buck. I always pick the one that recognizes the clients fear/concern and allows for further conversation. I recognized that you can never just answer a client's question, you have to observe and recognize their concern for it and allow them to further express what they are feeling.
Now I'm getting some great feedback. Just what I needed. Thank You!
