Hi there:
I am new to the MDS Title and really need some help with organizing charting for Nursing Staff. I got a great flow sheet from the AANAC for CNA's and would love to share this if anyone wants it. The flowchart is on Adobe Acrobat and I think I can download it for anyone here if you would like.
I am hoping to get some help with assisting staff with charting savy We all need help with support documentation for residents tht matches MDS and Staff/Nurse Charting.
Any and all help is appreciated.
I always believed that all of the floor nursing documents should be derived from the MDS. I would suggest making/editing flow sheets...instead of monthly summaries. Nurses could just check off what is going on, and you don't have to worry about use if wrong terminology.

I'm wondering what's the point of having the CNAs do the caretracker and put in exactly how much help the pt needs with ADLs, transfers, dressing, bathing, continent/incontient, eating, etc. etc. if I'm supposed to also write this exhaustive note detailing how much help they needed with ADLS. Do we really need to write continent of bowel and bladder 3x a day in the nurses note for 100 days when the CNAs are documenting the same thing?
I do focused notes based on admitting diagnosis. I don't write out stuff I've documented on three other flow sheets longhand in my note.
