Has anyone applied (or planning on applying) to the RN program at LAHC college? Is there a waitlist that you know of? Any comments, tips, or general info on this school is much appreciated.

which part are you having trouble with?

What is your question about the insurance form? It says you can apply online at Professional Liability Insurance for nurses, nursing medical malpractice. Don't forget to select student nurse/cost $20 (unless you are an lvn). Otherwise, tell me which part you need help with. :) :D All we need to turn in is the first page of the policy that states the coverage amounts.

on the front page(N.P.L.Insurance Application page),

Number 3, 8, and 9-neither self nor employee I think.

and Back page, Authorization and Release all part, First line is my nme and Name(Hospital/Agency)--> Clinic Hospital name? or LAHC?

I suggest you do it online its a LOT easier and quicker. Click on that link.

Also, on the first page, Should I check "Yes" part?

I tried ,but there are some differnties

its a little different, but i think its ultimately the same. but if you HAVE to use the "fax" version, then yes check "yes".

The coverage amount is correct. The price is 50 cents higher than the paper! Bummer!

I was just looking at that question...from what I see, it's good for a year, so you might as well put today??? I don't know. :confused:
