Looking for a great ICU flow for RNs to organize their shift

Hi everyone...Thanks for taking a look ?

I am looking for a flow or as some nurses call it a "Brain sheet". Basically what I need is a worksheet to follow throughout my shift. We don't have a Kardex (well we do but it is never updated so we all use it as scrap paper). So I am wondering what do you guys use to organize your shift?? I need something to work off of, which would include times to give meds, lab draws, vent settings, ivf's etc. I want to design my own so I am looking for any good ideas out there.

Ps. I am extremely anal so the more info the better and I work in a SICU.

Any examples would be greatly appreciated.......

Honestly, I just go by memory. I work with people who lose their minds if they misplace their "memory sheets". I orient a lot of new nurses. Some make up sheets based on body systems and some just jot down important notes. Meds are on the MAR, vent settings are something I remember, vitals, reding ect. If you are doing in depth assessments and making a mental not of what is going on it is usually to remember ..........my drips are set on x, my vent is set on y, my patients vitals and rhythm are x, and so on. We use shift kardex sheets for dates of procedures and to update drips during report sometimes. We also do 12 chart checks as well.

This is the "brain" that I use. I make a double sided copy, so I only have one paper in my pocket. Hope this helps!

I have this problem, so to solve it I print the worksheet out on a colored paper which makes it highly visable to me and others. That has reduced this problem.

Here is my report sheet that I currently use when I'm in the ICU as well. I've had a number of people PM me and sent copies via email so here is a copy of the one I like!

I'm attaching one that I use and like. It has slots for each hour of my shift that I write in things that are due like meds or timed labs and I write HR, BP, RR, Temp into the boxes Q4...it helps me stay on top of what I should be doing throughout my shift.

I am attaching the one that I use. There is a front and back side to this and I am able to fit my 2 sheets of paper for my 2 pts easily in my pocket. I cannot take all the credit for this, I actually adapted a sheet I found on allnurses that I used when I was on med-surg and then modified it when I went to ICU. I circle the pertinent info and write other things in the blank. Example, circle NGT and write LIS. Often I circle what I am told in report and then put a check next to it during my assessment if I agree or if there is a change I write it down in the report section. Under admit info - I put outlying facility they came from, ground air etc. My back page is the details and I circle the hours I have meds scheduled and accuchecks scheduled and this is where I write down temps and if titrating what their VS's were. I just got a job in CVICU- based off of my prior cardio/step-down sheet, this is the more detailed 2-sided sheet I'm trialing (I personally need the large time grid to visualize time/tasks in relation to others... I have AS so I need to write most things down to remember...unless they're numbers)
