Long term care nursing learning plan

I have been trying to think of a learning plan topic related to LTC...

So far I have thought of

inter-intra professional collaboration...but don't even know what learning questions I could use...

RAI-MDS...

Leadership development...

Palliative...

3Ds (dementia, delirium, depression)

....I don't know what a good topic would be...nor what learning questions to use...

suggestions? ideas? advice?

How about the transformative nursing home model, or client-centered care planning? I had to read this book in my geriatric nursing class. It's a pretty amazing concept!
