Kaplan Trainer 5 - Question 68 (Digoxin)

Published

So if a patient has a plasma level of 2.0 we should withhold the medication??? I figured if the therapeutic level is 0.5 to 2.0, then 2.0 is still therapeutic, especially since it's a maintenance dose. If the dose is withheld, wouldn't the level drop?? Please share your opinion, but I think this question is poorly written.

  • order for digoxin (Lanoxin) 25 mg PO daily
  • apical pulse 68/min
  • respirations 16/min
  • plasma digoxin level 2 ng/ml

  • which of the nursing action is appropriate?

  1. 1. Give the medication on time.
  2. 2. Withhold the medication; notify the physician.
  3. 3. Administer epinephrine 1:1,000 stat.
  4. 4. Check the client's blood pressure.

(1) medication should be withheld

(2) correct-therapeutic plasma level of digoxin is 0.5-2.0 ng/ml

(3) not a correct action

(4) assessment, does not address the issue of the elevated blood level of digoxin

yep, i'm filipino... and super stressed....... about 2 weeks away... and i'm contemplating on enrolling at Rachell Allen for final coaching... :( my mind is all over the place

haha.. u in the phils or in the states?..good thing you still have ample time .. and you get good rates.. im just on the 55-65.. rating on the q trainers.. haha.. the anxiety is killing me.. is it true that NCLEX is easier than the NLE? i took and passed the NLE last july..i was confident that time.. but the NCLEX just gives me the creeps..

Three pages of comments and I was the only one who noticed the dose was way too high? I'm glad I'm not on dig if all y'all are passing meds!

i'd like to think that i would have noticed that:eek:, so i went back and looked at it again...and discovered a small goober of something on my screen in exactly that position. call it an exogenous decimal point.

i feel much better...and am not seeing things all [color=#ffd700]yellowy[color=#00ff00]-green, either.:D

i'm here in the phlilippines.... i havent taken the NLE, but from what I hear, it's much harder... mainly because there are already 250,000 unemployed RNs, so the BON here feels like they should make the exam extremely difficult. It's also unpredictable and taxing because there are 500 questions. Not to mention the fact that some of my classmates had to take their exam in a non-airconditioned room, so the testing environment adds to the anxiety.... i think NCLEX is more application while NLE has alot of recall questons

crazy that this says 25 mg of digoxin...its supposed to be 0.25 mg right? I just got a nclex review question that made the same mistake. I was like...does that say mcg? no, is says 25 mg.
