Sorry if this is a dumb question but I have trouble with this. Does anyone know a good site for recommended med diultion and infusion rate chart for commonly used meds in the NICU?

Our pharmacy has compiled a big binder with all we need to know about our meds. There are very few we dilute ourselves now, as our pharmacy does it all for us.

That's what we do as well. We have standard dosages, compatibility charts, which meds need levels drawn- the works. The only things I ever dilute tend to be the really, really teeny doses of morphine, ativan, etc. It may be entirely unnecessary, but I can't make myself believe that 0.05ml of fluid is really getting anywhere. It still may not get anywhere when I mix it with 0.5ml of NS, but it makes me feel better. :)

Neofax is the NICU bible. I nit are the proper dilutions for all of your meds.

Too much fluid, even a cc here and another cc there can quickly add up. If a 24 weeker who has minimal kidney function at best gets an extra 10-15 ccs each day It can be a huge deal as he turns into a little bloated water baby.

Underdilution of extremely alkalotic meds can cause phlebitis or infiltrations.

If your unit does not have a Neofax suggest that they get one.
