I'll be kick out of the nursing program???

Hello everyone......
iam new to the board and i really didnt expect my first post to have a topic like that but i might be kick out of my nursing program!!!! the hospital where i do my clinicals is filing an incident report on me.........:imbar cuz i made an error when i document... now i am afraid my school is gonna take action on me!
Does anyone have similar or heard of similar experiences? am i gonna be kick out of my nursing program? i could not eat or sleep since yesterday.... please tell me some good news or please tell me what i should or can do?

How they handle the error will depend on what type of error it was and what type of effect it had on things. Were you acting in the role of a student when you made the error?

Incident reports sound a lot scarier than what they usually are. We have to fill the things out for anything that isn't routinely expected. For instance, I have to fill out an incident if I have a pt with the tiniest paper-cut on their finger. Incident reports are supposed to be for quality improvement and risk management.

I wish I could be of help. I suppose it really comes down to what they are saying you did or didn't do and what the outcome was. Lots of good advice on this board though. Good luck to you.

In addition to what Froggylegs said, an incident report is just that, and it does not go into the patient's record. The person ultimately responsible for your mistake, and who carries the largest burden is your instructor. I would suggest you talk to the instructor, to see what can be done to correct your mistake, and so you don't make the same mistake again.

hello everyone, thanks for all of your responses! how should i start? well it's the dumbest mistake... ok i have some problems telling Lt to Rt, Rt to Lt!!! i know... maybe i shouldnt be a nurse afterall anyway i am usually ok it's just that i would need more time to think about if it's Lt or Rt but i guess that day i didnt realize that i wrote down Lt. instead of Rt. where the wound is, inaccuately. the hospital filed an incident report and my instructor is going to see me next week. now iam much worried, cause i've heard stories where the other nursing student had incident reports filed by the hospital and they were out of the program!!:uhoh21: i know the mistake seems little but it's actually a big problem for me in the future if i am out there in the real world as a RN, but i am hoping i won't get kick out of the program cause i've spent so much time and efforts! :crying2:

so this is the whole story guys.......i know, iam so dumb:o

You know what? I have had trouble remembering left from right my entire life. I just have to think about which hand I write with (right) and use that as my point of reference. This won't help in regards to your current situation, but it might be of assistance in the future in identifying one side from the other. It doesn't mean you're dumb. I know I'm not dumb (although I have to tell you, I developed this coping method in 3rd grade when a particularly unpleasant teacher told me that I was). It is just a "glitch" in how my brain works, I guess.

My goodness, I would HOPE they wouldn't kick you out based on that! Geez. I can't believe they're going to do an INCIDENT REPORT over it! Did you actually CAUSE the wound or did you just document it incorrectly? Why can't they just do an addendum note in the chart?

I wonder if one of their own did it would they write up themselves or their peers. I doubt it.

It was really hard to get the hand of that part. It can be very confusing. My right is their left. I still find myself turning to figure it out. The incident report could have been done as a near miss. We had to do one with a student this semester because of a mix up with insulin (humalog vs humalin). It helps them with planning in risk management. You may get written up, a verbal warning, but I highly doubt you would be kicked out of a program for that. -Andrea

I've seen many nurses and docs accidentally switch the extremities around when they document and it's correctable on paper. Now, going to the OR and having the wrong extremity operated on - that's another issue! Try not to worry too much, I'm sure things will be fine!

Don't feel too bad, I have trouble with right and left too. I think it's genetic, my aunt is the same way. They say look down and see which of your hands makes an L shape when you spread your thumb away from your fingers. I don't find that all that helpful, but you might.

Out of curiosity, any others with the right/left problem have early dance training? I sometimes think it's related to the mirrors and the teacher's left being your right (those two factors in combination).
