I encountered a patient very recently that was on a dose of Keppra I'd never seen -- 2500 mg, BID. Yep -- 5 grams of Keppra daily. And he's still having seizures! Yikes. Has anyone ever had a patient on this much Keppra? I just stood there with his pill bottle, going blink ... blink ... blink ... "Really?"
Edited to add: typical max is 3000 mg daily, according to Epocrates.

My pt. is on keppra but I dont recall the dose to be that high.

I gave a loading dose of 900 yesterday to someone actively seizing. That sounds like an enormous dose.

wow, perhaps it's time for a new med since the Seizures persist.... ?
I think this is the last resort - they've gone through all the others. Sad.
My resident is on 1000mg Bid.

i take 500mg bid plus 300mg neurontin tid. my neuro said i can take the entire 1000 mg of keppra hs but it makes me too loopy if i do it that way. with that combo and dosage, i have essentially no side effects at all. before, on 1200 mg tegretol plus the neurontin, i was always sleepy and, according to my husband, irritable sometimes.
