I just started with peds and I am still shadowing. I was wondering how much Xylo do you mix with Rocephin?

It should be right there on the bottle. If it is not, then call the pharmacist, he will be able to tell you. I know that the last time I did it, it was printed on the bottle. It might say mix with NS, but I was taught that you can substitute NS with lidocaine.... Definitely call the pharmacist :)

I'd check with pharmacy also if there isn't a standing protocol for it. We had a protocol and it was taped on the wall next to the Pyxis.

Also check and see if you need an order to use the lidocaine. I've worked at a couple of facilities that we couldn't just substitute, had to have the order in place.

We use lido 1% plain - 1ml for 500mg Rocephin and 2.1ml for 1 gram Rocephin. And then of course figure the amount in ml's from there. Our bottles state this, but I can't remember what brand our ED carries.

How old are the kiddo's are you using it on? I had a pediatrician way back tell me not to use it on little ones as you are exposing them to it unnecessarily. He reasoned it as you could potentially could risk allergy down the road, also it is not a benign drug. I have since followed the if they can/should walk rule.

Ask the MD first if you can mix Lidocaine with Rocephin. Some physicians may say it is O.K. to mix it but some may not. I wouldn't mix anything unless I ask the MD first.
